Tools for Making a Smooth Transition

Below are suggestions for making the transition to a new early intervention provider. The service coordinator with whom your family currently works will contact your new provider before you leave your current location and help make sure all necessary preparations are completed, including identifying payment options if a fee for services is charged by the new provider. Early intervention services that are typically provided at no cost include:

  • Child find
  • Evaluations and assessments to determine eligibility
  • Service coordination
  • Development, review and evaluation of individual family service plans
  • Implementation of procedural safeguards

To help track your activities and progress, a checklist of the steps below is provided for your convenience.

Suggestions to consider before you leave your current location

  • Review general information about your new location at MilitaryINSTALLATIONS.
  • Notify your current service coordinator that you will be leaving and where you will be going. This will allow time to prepare for your transition.
  • Use a three-ring binder or other type of notebook to organize your child's records and other pertinent information.
  • Make sure you have paper copies of your child's:
    • Most recent eligibility report that indicates your child is eligible for early intervention services
    • Current individual family service plan and any progress reports, including service summaries from speech/language therapists, physical therapists, occupational therapists, etc.
    • Medical reports and developmental history
    • Inoculation records
    • Prescriptions for medications including dosages

Note: States often have different eligibility criteria for services and your child's eligibility may be re-evaluated at your new location. Having the above information with you will help the new provider complete the process in a timely fashion.

  • Coordinate with your child's service coordinator and write a brief summary of your child's current program to share with the new provider. A convenient form is provided for your use.
  • Use the State, Early Intervention Provider and Installation Information section in the directory to identify the early intervention provider at your new location and learn more about the agency.
  • Give your current early intervention provider the contact information of the new early intervention provider to facilitate the transfer of information.
  • Keep the name and contact information of your child's current early intervention coordinator in case you or the new early intervention provider have questions after you arrive at your new location.
  • Contact the Exceptional Family Member Program to learn more about early intervention services at your new location. A list of sample questions to ask is provided and can be printed for your convenience.
  • Ask your current service coordinator to contact the early intervention provider at your new location to learn more about their eligibility requirements and services. A list of sample questions is available for your convenience.
  • Write down the time, date and person of anyone you speak to regarding your child's transition, as well as some brief notes. A printable worksheet, developed by the Center for Parent Information and Resources, is included for your convenience.
  • Review your educational rights. Examples of parental rights and safeguards under the Individuals with Disabilities Education Act from various states are available at The Early Childhood Technical Assistance Center's Rights and Safeguards page. Your current service coordinator can also answer any questions you may have.
